We accept 3rd party on a case by case basis. 
1- You must book online and a card on file is required. 
2- Rates are higher for this and you are responsible for all additional payments not covered by the settlement when your Lawyer settles your case. In the event the other persons insurance does not pay for your massage therapy treatment, you will be responsible for the entire cost  remaining for services 30 days after completion of treatments. Late fees will be charged for non payment 
3- You must have a valid Credit card on file and sign an agreement that you accept responsibility for payment in full if your lawyer does not send forth payment after your case settles
4- After 3 months of treatment, you will be responsible for making monthly payments of $200 per month. Lakeside Massage and Wellness Studio receives the total remaining cost of services Paid in Full when your case settles and will deduct your monthly payments total from the total balance of treatments to send the remainder owed to your lawyers. 
5- We will need to verify both insurance parties information. 
6- We will need a claim number 
7- Third party billing requires a Doctors, PT's, or Chiropractors referral/ prescription for massage

* First time appointments please schedule a 90 min session so there is enough time to go over the details of your accident, assess the treatment plan, and complete any and all necessary ROM testing m, full assessment and paperwork needed to move forward with treatment 

We do place a $5.00 credit card processing fee that is non refundable for each time you book an appointment appointment to verify that the credit card you leave on file is a valid credit card in the event you violate any of our cancellation policies or promise to pay agreements. This will be billed automatically for each appointment and does not go to the cost of your massage. It is to cover our credit card fees that we pay to run and verify your card you leave on file is valid. This amount is non refundable and will be required for every appointment booked even if you have to cancel your appointment and reschedule on time
