We do not bill this modality for personal insurance. You will need to pay at the time of service and send a request for reimbursement to your insurance provider. All costs will your responsibility and will be billed for within 24 hours of receiving treatment to the card on file unless otherwise agreed upon by you and Sankalpa Bhogah Lakeside Massage & Wellness. We will not refund the amount charged back to you and you cannot receive a credit from lakeside so please make sure that you, your attorney and lakeside massage and wellness has a clear understanding of the medical massage payment plan prior to your appointment.
All details for our billing will be emailed to you with your intake form. It will outline your promise to pay agreement in full if you have a plan set up. Please read it and sign it before your appointment.
We do place a $5.00 credit card processing fee that is non refundable for each time you book an appointment appointment to verify that the credit card you leave on file is a valid credit card in the event you violate any of our cancellation policies or promise to pay agreements. This will be billed automatically for each appointment and does not go to the cost of your massage. It is to cover our credit card fees that we pay to run and verify your card you leave on file is valid. This amount is non refundable and will be required for every appointment booked even if you have to cancel your appointment and reschedule on time
